We are experts in delivering innovative integrated health and social care to ensure people accessing our services achieve the best outcomes. We specialise in supporting people with the most complex multiple disadvantages, including mental ill health, physical and learning disabilities, substance use and alcohol dependency, victim/survivors of abuse, trauma, or exploitation, people with experiences of homelessness, and those with histories of the Criminal Justice System.
With over 500 staff across 44 services and 602 accommodation units, we provide thousands of people with essential support and care in temporary supported housing, mental health drop-in centres, community floating support, criminal justice settings and hospitals.
We empower people by building powerful partnerships and creative solutions that bridge gaps in provision and aid recovery, reablement, and resettlement.
We believe good care, support, and well-planned services make a difference and improve lives."
